Athletic Physicals Grades 7-12

On July 14th from 2:00-6:00 PM we will have physicians from UPMC doing athletic physical exams for Sharpsville student-athletes in grades 7-12 for only $10.00 (Cash Only)! We'll have it right at the high school. This is a great opportunity for your student-athletes to get their physical for the school year.

Before a doctor will see you on the 14th, the attached physical forms need completed through the bottom of page 5, with parent/guardian signatures on pages 2–5. Please bring the completed packet with you.

Important Update: As we prepare for the upcoming season, Sharpsville will be moving to a new operating system as Big Teams was bought by ArbiterSports. I hope to have the updated system ready to use as early as next week. Thank you for your patience.
