 Welcome to the 2014-2015 school year!  

Please remember to complete the back of your child's emergency form indicating which if any over the counter medications you permit them to take during school hours.  If this section is not complete or no medication is checked, then your child can NOT receive any Tylenol or any other over the counter medication during the school day.  If your child is going to require prescription medication during the school day, please note that there are (2) specific forms that need completed prior to any administration of these medications.  These include but are not limited to; asthma inhalers, ADHD medications and emergency epi pens.  These documents can be found on this page under downloads.
 
Per PA State Law, all children in grades K, 6, and 11 are required to have a physical exam on file.  Also, all children in grades K, 3, and 7 are required to have a dental exam on file.  You can download these forms from this page as well. 
 
We do hearing, vision and height/weight screenings throughout the school year.  Please note that hearing screenings are done on children in grades K-3, 7 and 11 grade.  Vision screenings are done on all students K - 12.  Height/Weight screenings are done on all students K - 12.  You will ONLY be notified if your child fails their hearing or vision screen.  Passing results are NOT sent home.  You will receive a letter regarding their height/weight results. 
 
Please know that we have many students who visit the Nurse's office throughout the day and we do not call home with every visit your child makes to our office.  We assess their injury/illness and determine if parents need to be notified.  Our policy states that ALL children will be sent home for a fever of 100.5 or higher.  Of course there are exceptions to this and we use our professional judgment in deciding the best course of action for your child.  If you have specific things you would like us to consider regarding your child, please let us know so we can be sure to accommodate your requests.
